Forms For Photographers

Below are the forms that Photographers who are participating "drawing type" events will need to conduct their no-cost portrait sessions for winners. If you are joining us for one of our "full-day" events, you will generally not need these forms as we will have them available for you on the day of the event.

Please print or download the Model Release/Permission for Photography form below and ask each family/individual to fill out and sign for each session you conduct.

Please print or download the Copyright Reproduction Release below and fill it out for each session you conduct. Please provide a filled out copy to each individual/family when you deliver their free print and CD/DVD. Also, there is a .txt file version which we would ask that you please download and include on each CD/DVD you provide to them.

Below is a CD/DVD label which you may use for the CD/DVD you provide to each individual/family. We ask that at a minimum, you include the www.GivingBackThroughTheLens.org website and your name on each disc you provide.
